Special Diet Requests

  If your child requires special diet considereations or needs lactose-free milk,
      please review the following information.
 

First, review the “Special Dietary Needs Policy” to deteremine which request form needs to be filled out. 
 
Special Diet Medical Statements need to be filled out each school year. 
 
Turn in special diet requests to your School Nurse or the Nutrition Coordinator in the Food and Nutrition Services Department.
